Meet Up and Chow Down - Site Interest Form

Thank you for your interest in bringing Meet Up and Chow Down to your community in 2026!

Food for this program is purchased through the Minnesota Department of Education/United States Department of Agriculture's Summer Food Service Program; in order to host a site:

a) certain requirements must be met for food and youth safety as well as volunteer capability
b) UWNEMN must apply for state approval for all sites on an annal basis
c) the state must review and approve UWNEMN's application

Please fill out the form below by March 15, 2026, to be included for consideration with UWNEMN's other Meet Up and Chow Down sites.
